Plymouth Swim Club Membership Media Release Form
As part of your Plymouth Swim Club membership, we request your consent regarding the use of photographs and videos that may feature you or your family members. This release allows Plymouth Swim Club to use media captured during club events or general activities for promotional purposes.
1. Media Use Authorization By checking the box below, you agree to allow Plymouth Swim Club to photograph and/or record video footage of members, including yourself and your dependents, in the club’s facilities and during club activities. This media may be used by Plymouth Swim Club for marketing purposes, including but not limited to:
- Club publications
- Website and digital platforms
- Social media channels
- Promotional materials or events
2. Non-Exclusive, Royalty-Free License Your consent grants Plymouth Swim Club a non-exclusive, royalty-free license to use, edit, copy, publish, and distribute these materials in perpetuity without compensation or further permission from you. This includes any media featuring incidental appearances by you or your dependents.
3. Request for Media Removal Should you wish for any photo or video featuring you or your family members to be removed, you may submit a written request to Plymouth Swim Club via email. Upon receipt, Plymouth Swim Club will promptly remove the specified media from active promotional channels.
4. Limitation of Liability By agreeing to this release, you acknowledge that Plymouth Swim Club has no control over media shared or re-shared by third parties. While we will make every reasonable effort to honor removal requests on platforms we control, Plymouth Swim Club cannot guarantee the removal of media previously shared on third-party sites.
You understand that your consent is a condition of membership and agree to the terms outlined above.
